Certificate of Immunization Status (pdf)
This verification of immunizations is required to be on file for all EIA students.

Student Records Request (pdf)
This form needs to be completed and submitted to a student's resident district (if not Evergreen S.D.), so the student's records may be transferred to EIA.

Application for Attendance in Nonresident District (pdf)
All students wishing to attend EIA but who live outside of the Evergreen School District's boundaries must have this form completed by their resident school district and submitted to EIA before being enrolled in courses.
